How does it work?
- Online registration - in order to activate the MyHealthPortal portal, each and every Lifeline Plus policyholder must register online (for registration click here).
- Application to open an EHR file - Any Lifeline Plus policyholder, for everyday needs and from any where is entitled to apply to open a personal EHR via a variety of communication methods:

- Clarification - In case an additional explanation about the service or the procedure is needed, the MyHealthPortal team is available via a variety of communication methods:

- Verification - The EHR service as well as the other MyHealthPortal services are available exclusively for Lifeline Plus policyholders. For each application, the MyHealthPortal system shall verify the eligibility to obtain the service. A service message shall be automatically sent via SMS or e-mail indicating approval.
- Application form and SMS Approval- once the MyHealthPortal system approves eligibility, the service can be provided. Opening an EHR can be done by using the designated online form. Once the application form is submtted an SMS message or email shall be sent as an approval notification.
- Evaluating the application form - The MyHealthPortal team shall evaluate the application for the service according to professional parameters. At this stage the applicant will be contacted by a MyHealthPortal team member in order to schedule a meeting. It is possible to conduct the web visit via a variety of communication methods: online chat, online video conference, SMS message, skype, e-mail, phone and more.
- The Questionnaire - In order to open an EHR, a member of the MyHealthPortal medical team shall use a specifically designed medical questionnaire. The main idea is to collect accurate relevant medical information. The questionnaire shall cover the following features:
- Applicant demographics.
- Health history (personal and family).
- Past and current medications.
- Past and current acute medical problems.
- Chronic medical conditions
- Vital signs (pulse and blood pressure).
- Risk factors (smoking, alcohol consumtion)
- Allergies.
- Immunisations.
- Life style indicators.
- Laboratory data - if available.
- Radiology reports- if available.
- Medical approval- Once the questionnaire is completed; the health web page is transferred to the MyHealthPortal medical director for their cross check and approval. The questionnaire is approved only when the MyHealthPortal medical director confirms that it was completed according to the highest medical standards and that there is no vital data missing.
- The Health web page (EHR): Once the questionnaire has been completed and approved by the MyHealthPortal Medical Director, the updated health web page shall be uploaded to the MyHealthPortal system. A universal password shall be sent by SMS to the applicant. The health web page is accessible only by using a login name and password.

- EHR ongoing update- The EHR is a ongoing service. According to the applicants preferences and consent, any previous and future interactions with the MyHealthPortal portal will be added to their health web page. Any future services given by the MyHealthPortal team will be based on the health web page (EHR). It is advisable to update the MyHealthPortal team of your current health condition, in order to receive the best care possible
